For the Musical services as stipulated above, the PARTY OF THE FIRST PART agrees to pay MUSIC MADNESS by cash, money order, or PayPal, the TOTAL SUM OF $ as follows: Deposit of $100.00 to be received When submitting this online contract: BALANCE OF $ payable on or before date of engagement. PayPal payments for the balance must be made before the date of the function.

Additional playing time, when available, is at the rate of $40.00 for each additional 1/2 hour (thirty minutes) or any portion thereof.

Is DJ included for dinner? Yes No

Type in any special requests or comments:

1. The agreement of MUSIC MADNESS to perform is subject to proven detention by illness, accidents to personnel, equipment or transportation of MUSIC MADNESS, riots or other civil disobedience, strikes, acts of God, or other conditions beyond their control. In any event, the deposit will immediately be refunded to the PARTY OF THE FIRST PART.
2. Except as provided in Paragraph 1 (above), in no event shall the PARTY OF THE FIRST PART be entitled to a refund of the deposit.
3. PARTY OF THE FIRST PART and MUSIC MADNESS further agree that this Contract is not subject to cancellation (whether or not the engagement is held) unless both parties hereto have agreed to such cancellation in writing.
4. For the true and faithful performance of any and all covenants and agreements herein mentioned, the PARTY OF THE FIRST PART hereby agrees that MUSIC MADNESS shall be allowed to retain the aforementioned deposit in the full amount indicated as liquidated damages in the event of default by PARTY OF THE FIRST PART.
5. Except as explicitly provided in the Contract, MUSIC MADNESS accepts no other liabilities either expressed or implied.
6. In the event of a default by the PARTY OF THE FIRST PART in the payment of any sums due by the PARTY OF THE FIRST PART under this Contract, the PARTY OF THE FIRST PART shall pay MUSIC MADNESS a reasonable attorney's fee and any other costs incurred by MUSIC MADNESS in the collection of same together with interest on such sums from the engagement date to date of payment in full at the rate of 1.5 percent per month.
7. No date shall be guaranteed without a Deposit returned to MUSIC MADNESS.
